The "My Life a Full life smart passport system" has been designed specifically to allow a new way of delivering health and social care services on the island to improve the lives of people needing care and support and empower them to remain healthy and well.

An integrated Primary and Acute Care System is achieved by joining up General Practice Services, Hospital Care, Community and Mental Health Services through an electronic integrated Passport System.

The Passport System enables the development of a true person-centred, co-ordinated health and social care service for the 140,000 residents on the island. This will support and validate better outcomes for people and allow co-ordinated and defined work within the local community to build capacity and resilience of people, families and carers.

The Passport allows a single point of access putting people at the centre of their health for all related activities encouraging the patient to be actively involved in their own care needs through integrated community activities, helping people to care for themselves and assisting individuals to overcome crises to return to living as full a life as possible.
